"Should I focus on AIQ or my state counselling?" is one of the most common questions candidates ask, and the honest answer is: you should run both, but understanding how they differ helps you set realistic expectations and order your choice lists sensibly.
Seat pool size
All-India Quota only draws from 15% of government seats nationwide plus central institutions — a relatively small, extremely competitive pool where every seat is contested by candidates from all 25+ states. State-quota counselling draws from the much larger 85% of government seats plus most private colleges in that state, but only among candidates eligible for that state's domicile quota — meaning your effective competition is a much smaller, more localised pool.
Why the same rank performs differently
Because AIQ pools candidates from the entire country, a rank that feels ordinary nationally can still be highly competitive within a single state's own counselling, where you're only up against candidates from that state. This is the single biggest reason a state-quota government seat is frequently the more realistic route to a govt MBBS seat for many candidates, especially outside the handful of states with the largest number of NEET aspirants.
Fee and bond differences
AIQ government seats and state-quota government seats are both heavily subsidised compared to private/management seats, but the exact fee structure, hostel costs, and any compulsory service-bond obligation are set independently by each institution or state — they are not standardised nationally. Always check the specific college's fee structure and your home state's bond rules rather than assuming they match a friend's experience in a different state.
A simple way to decide priority
If your rank is strong enough to be competitive at the national level, weight your AIQ choice list toward the strongest institutions since that pool includes AIIMS/JIPMER/central colleges unavailable through any state process. If your rank is more realistic for your home state specifically, put real effort into your state choice list — it is very often where a govt seat is actually won, particularly in your home state's own reserved-category quotas.
